TRANSCRIPT REQUEST FORM - COLLEGE CREDITS FOR ITALIAN STUDIES

Upon completion of your Babilonia courses, an original transcript of your course grades can be sent to the Registrar and/or faculty advisor at your college or university. Submit a written request to the Babilonia office including the information requested below. One free official copy of your transcript will be sent. An unofficial transcript will be given to you upon completion of study or sent to your home address. The charge for additional official transcripts is five Euros which can be brought along with this form to the Babilonia office or faxed to: 39 0942 23441 Attn: Alessandro Adorno.

All independent study credits are awarded at the discretion of your home college or university. Please contact your registrar and/or faculty advisor upon return to the U.S. to verify that the transcript has been received.
